Expanded In-Home Services for the Elderly

Living at home is a goal for many seniors like you. Nevertheless, as we age, we may find it harder to complete the most basic daily tasks like laundry, cleaning, and cooking. With the Schenectady County Expanded In-Home Services for the Elderly Program (EISEP), older adults 60 and older can receive assistance with daily living activities. This program is funded by the New York State Office for the Aging and available to eligible seniors.

What Services are Available to You?

Finding help and getting connected to services to support you or a family member can be difficult.  The Expanded In-home Services for the Elderly Program can help you or a loved one navigate an array of services and benefits that you may need. You'll be assigned a care coordinator who will help you with your initial assessment and reassessment, create a person-centered care plan, provide referrals for needed services, check in with you and provide ongoing assistance and advocacy as needed.

Bathing, hygiene, grooming, dressing, mobility assistance, and transfers.

Light housekeeping and cleaning, laundry, meal preparation, grocery shopping, and daily errands. 

Am I Eligible?

Must be 60 years of age or older

and

Must have a need for assistance in the area of daily living, such as bathing, hygiene or dressing and/or at least two instrumental activities of daily living like laundry, light housekeeping, meal prep, or shopping and errands.

Do You Receive Medicaid?

If you recieve Medicaid, in-home services may be provided to you through other programs and not the Expanded In-Home Services for the Elderly Program.

Does the Program Cost Money?

 

The Expanded In-home Services for the Elderly Program is a cost share program where you may be required to contribute a fee anywhere from 0% to 100 % of the total cost for services.  Your cost share is determined based on your monthly income.  Certain household expenses are taken into consideration when determining cost share.

To arrange for an assessment call the NY Connects of Schenectady County at 518-382-8481 x1. A NY Connects intake specialist will pre-screen you for eligibility. A case worker will be assign to you.
